Our office is centrally located in Mission Viejo one minute from the 5 Freeway at Oso Medical Plaza. 

The office is open Monday -Friday. Normally we are closed either Thur.or Friday. We see patients about once a month on Saturday mornings.

Dr. Addonizio  accepts many  insurance plans including a number of PPO plans.

Please bring your insurance information on your initial visit.
